This is too frequent for pilot error. 

Please describe EXACTLY what you did in sysinstall to get this error.

> wd0s1-2=swap 65536 none
> wd0s1-3=ufs 65535 /var
> wd0s1-4=ufs 0 /usr

Huh? What's the number?
