This is the clue: (WW) RADEON(0): config file vrefresh range 40-170Hz not within DDC vrefresh= =20 ranges. Try Option "IgnoreEDID" "on" in Section "Device". However, make extra-sure your monitor can *really* do 40-170Hz.
