How about:

pkg_info -oa | grep /

You can feed that to portinstall, or such:

pkg_info -oa | grep / > manifest.txt
<... copy manifest.txt to your new system ...>
cd /usr/ports/sysutils/portupgrade && make install && rehash
portinstall -p `cat manifest.txt`
