Without any information, I would guess from the fact that this is a Apache web server that you are exceeding the virtual memory on that machine and you do not have the VM patch that allows the correct action of terminating processes in this event. Please give more information of the symptoms, pertinate logs entries, OS level, RAM size, swap, etc. --mark tinguely.
