Okay, I figured it out.

The problem was using the default mfi driver instead of mrsas.  mfi usually
works but occasionally seems to go into a mode where it stops flushing data to
disk.  This of course cause all sorts of hilarious filesystem corruption.

An additional complication was that this server (still) suffers from the
"shutdown hangs after freebsd-update" described here:
