The manpage suggests
"|exec /usr/local/bin/procmail || exit 75"
I'm not sure whether that's the problem or not.
My .forward file has always been
"|IFS=' '&&p=/usr/local/bin/procmail&&test -f $p&&exec $p -Yf-||exit 75 #mcglk"
which was probably originally in old procmail documentation, but which works
great.
Other than that, all your permissions look fine.
