I had the same problem, using sendmail and can not move to other MTA 
(postfix, qmail). I asked on another list the same and Jeremiah send me

We are very happy with this solution, I guess at least 90% of the spam is 
blocked now. Just one advice, the server will be very , very busy blocking 
and rejecting email so expect to have a little delay on the service but 
that it is worth it.
