Hi

BDE sent me a much better way of doing the #include cleanup, which I
have built upon.

This sorts out <sys/mutex.h>, <sys/lock.h> and <sys/lockmgr.h> being
included in some other "system" includes.

At some stage, <sys/condvar.h> will need similar treatment.

Also - PHK's clever script to remove unused #includes may need to
be invoked after this has a chance to settle down.

Please review and comment.
