What values are you using for base= and prefix= in your supfile?

Did you look at the file:
/usr/src/share/FAQ/extras/standard-supfile

Also when running sup can it should be run as root, or it should be able
to write to /usr/src [it needs to create /usr/src/sup/... for state
information].
