Underscores can't be a part of entities; use &man.fsck.ffs.8; instead. See doc/share/sgml/man-refs.ent for a full list. If there's an entity missing that you need, drop a note to doc@ and (hopefully) one of us can add it for you. There was some mention earlier that this document could be added to the 5.0-RELEASE errata. I don't think this the right place, because the errata is fairly transient in nature and the information in this FAQ is much more long-lived. People will be interested in this information for some time to come. I was in favor of it going into the Handbook as prose (i.e. not a Q&A format), however: 1) I haven't looked for an appropriate place and 2) I probably won't be the one doing the work. :-) I only gave your FAQ a 30-second glance, but it did a good job of distilling information from a whole bunch of places...nice going.
