Step 1) Create the device entries...
# cd /dev
# sh MAKEDEV cuac0 ttyc0
Step 2) Build the kernel with the device configured...
# cd /sys/i386/conf
# vi GENERIC (or whatever config file)
Add the following line just after the sio devices...
device cy0 at isa? tty irq 10 iomem 0xd4000 iosiz 0x2000 vector cyintr
# config GENERIC (or whatever config file)
# cd ../../compile/GENERIC (or whatever file)
# make kernel install
# shutdown -r now
