Here's how I run it:

# irs -c -y /dev/ptypv -d /dev/cuaa1

then point (e.g.) /dev/pilot at /dev/ttypv

Note that I use the cua device as opposed to the ttyd device.  Note 
that you can't open the ttyd device until carrier detect comes up (you 
will block on the open), but you can open the cua device.
