 Sounds like you didn't read /usr/src/UPDATING.

 I'm not sure when PAM became a part of the base system, but I do recall
tons of problems with early 4.x releases when people forgot this:

20010112:         
Important new FreeBSD-version stuff: PAM support has been worked         
in, partially from the "Unix" OpenSSH version.  This requires         
adding the following in pam.conf:  
        
sshd    auth    sufficient      pam_skey.so         
sshd    auth    required        pam_unix.so         try_first_pass
sshd    auth    required        pam_permit.so

 My suggestion is to check that your copy of /etc/pam.conf contains those
lines.
