Turns out that Robert Withrow <bwithrow@baynetworks.com> answered my 
question the day before I asked it ;-)

I added:

DisplayManager._0.authName:   MIT-MAGIC-COOKIE-1

to my xdm-config and now it works.  I guess the 4.0 CD-ROM doesn't have 
XDM authorization compiled in.
