The text here needs to exactly match the card ID strings reported by
'pccardc dumpcis'.  For your card that's probably:

"CreditCard Ethernet 10/100 + Modem 56"

or something very similar.  I believe that the maximum length that can be
matched here has been increased to something larger than 30 (CIS_MAXSTR in
/usr/src/usr.sbin/pccard/pccardd/readcis.h) after people pointed out that
cards existed with longer ID strings.  There's a PR to that effect
somewhere with my name on it...

Hopefully that will get it going; if not, fire off an email to
<freebsd-xircom@lovett.com> with the same info you included here.
