This is because BMAKEENV has DESTDIR=${WORLDTMP}, and bsd.{prog,lib}.mk have the magic of adding the -I${DESTDIR}/usr/include to CFLAGS if DESTDIR is set. I'm currently testing the following patch with the -CURRENT build on a -STABLE box.
