The biggest hurdle is actually deciding to dump the default MTA packaged 
with the base OS (Sendmail).  Sendmail works ok for the bulk of 
installs.  Most people make the change for a specific reason: be it 
performance, better UCE control, easier configuration, security, and so 
on.  I'll go out on a line and say people who do opt for a repalcement MTA 
don't go back to sendmail in many cases.

Qmail VS Postfix VS Exim VS Foo: download each and give a test drive.  I 
went with Postfix, which suited the bulk of my requirements well.  I've 
heard good things about Qmail as well.  Not too much in the way of Exim, 
however.
