Did your kernel and userland get out of sync?  At some point prior to the
RELENG_5_0 branch, the format of the superblock for UFS2 changed -- on
booting a new kernel, the superblock was upgraded, but older kernels and
boot loaders could no longer read it, so you had to make sure to upgrade
the boot blocks also.
