You need an if - endif for each knob.  Something like this.

.if defined(WITH_PAM)
CONFIGURE_ARGS+=        --with-pam
PLIST_SUB+=             PAM=""
.else
CONFIGURE_ARGS+=        --without-pam
PLIST_SUB+=             PAM="@comment not installed: "
.endif
