I believe you have to hand edit the \documentstyle line to remove an 
extraneous comma (,) for it to work with 2.09.  It (should) work fine 
with 2e but I have not tested it in a while.

But this is unrelated.  It means that tex either cannot find the format 
file, or the file is corrupted somehow.  I'm not familiar with the 
package layout of TeX so I cannot tell you precicely where they should 
be.  (I'm still running the TeX I built on 386BSD+patchkit, before the 
birth of NetBSD and FreeBSD.)
