I'm afraid I can't help with the rest, but I do know that you can't mix generic PCCARD support (ie. pccardd) with the 'dedicated' drivers such as ze and zp. I'm pretty sure one of the generic drivers -- maybe ep? -- will work with your card. However (there's always a however), you can't currently use both parts of a multifunction card simultaneously; that is, you'll have to eject the card, shut down pccardd, edit pccard.conf, reinsert the card and restart pccardd (or maybe reboot...) to switch between the modem and Ethernet. Sucks, I know, but trust me, people are working towards a fix for this.
