see /etc/disktab. i use this script to create floppies:
roman@freepuppy ~ 1021:0 > < ~/bin/newfd
#!/bin/sh
fdformat -f 1440 fd0.1440 \
&& disklabel -r -w fd0.1440 fd1440 \
&& newfs -T fd1440 fd0.1440
