I use the following arguments for its configure script: ./configure --prefix=/usr --sysconfdir=/etc/ssh --libdir=/usr/lib \ --includedir=/usr/include --infodir=/usr/share/info \ --mandir=/usr/share/man --with-tcp-wrappers --with-pam \ --with-mantype=man --with-md5-passwords I also set NO_OPENSSH=true in /etc/make.conf, to prevent subsequent overwriting of 2.9.9p2 by make world. The only caveat is that you need to recompile OpenSSH when the -STABLE branch updates its version of OpenSSL (unless you also build that yourself, of course; that may break other stuff, however).
