As you're using slices, the disklabel command needs a slice argument:
eg. da0s1 rather than da0.

It seems you may have a second copy of the boot manager installed where
the boot blocks should be.

If the proper disklabel command doesn't fix this, take a look at the
fdisk(8) output for the disk and verify that the slice starts in the
correct location: typically cyl=0 head=1 sector=1.
