Yes, that's true. However, bsd.{prog,lib}.mk include ../Makefile.inc, which
can be used to source /etc/make.conf if not already done.
But I hope there are cleaner ways.

Yes, they do. DOC_LANG for example.
However, doc.conf is the last one on my list.
