If you can easily reproduce this, it would be nice if you could compile a kernel with KTR, and KTR_MASK set to KTR_CT2. Then please wait until the problem occurs, and dump the trace buffer (using ktrdump is probably the easiest way to do this). The driver has pretty verbose traces in it, so I hopefully should be able to tell what's wrong from seeing the output.
