It is present in 12 as far as I can tell:

# find /usr/include -name stddef.h -print | more
/usr/include/c++/v1/tr1/stddef.h
/usr/include/c++/v1/stddef.h
/usr/include/sys/stddef.h
/usr/include/stddef.h

# uname -apKU
FreeBSD FreeBSDx64OPC 12.0-CURRENT FreeBSD 12.0-CURRENT  r325700M  amd64 =
amd64 1200053 1200053

tddef.h is listed in:

https://svnweb.freebsd.org/base/head/include/

and in (see: line 25 ):

My guess is something is odd with your environment.
