rm -R /usr/ports/distfiles

It doesn't get any less manual than that without cron. :)

You could write a script that reads /var/db/pkg and then searches for
files for distfiles _not_ in /var/db/pkg and removes them from
/usr/ports/distfiles. You might do this is if you want only to delete out
of date distfiles.
