Update port to v1.00 from 0.92.

Tested on FreeBSD 4.5 release with a couple of small PDFs from docbook output.

Removed the readonly patch (no longer applies properly).  But I also didn't create a new one as I don't think that type of thing can be stored in the ports tree itself could it?  Potential copyright prevention stuff?
