a white background usually hurts my eyes, as does the default taint of green
after installing windoze 9x

I tend to use the equivalent of "xterm -fg green -bg black", use either a
dark solid grey root window or "xphoon", lower the brightness and contrast
settings of the monitor, and do everything possbile to minimize onscreen
glare or bright sources of light in my peripheral vision.  Dark wall
hangings work well to minimize reflected light.

Ergonomically speaking, you should minimize the number of distinct colors
you need to actually focus on at once onscreen.  Has something to do with
refractive indices and focusing up close.  

Also there is great value in just taking a break every 10-30 minutes and
looking away from the screen for a few minutes, or getting up a shambling 
about.  That can help more than anything else.

A couple of lines from my .Xresources file;

XTerm*Foreground: green
XTerm*Background: black
emacs.foreground:green
emacs.background:black
