I manged to solve the problem, here is the solution:

pass in on em0 proto udp from 10.5.1.0/24 to port 69 rdr-to 10.0.0.11
port 200001
pass in on em0 proto udp from 10.5.2.0/24 to port 69 rdr-to 10.0.0.11
port 200002
