I have been investigating this issue using a QEMU-based reproduction
environment (FreeBSD 14.4). My traces show XHCI Error 18 (Context State Error)
during S3 bounceback. However, I’ve also observed that host-level resource
exhaustion (RAM limits) in virtualized environments can inconsistently trigger
this protocol failure. I am currently auditing the state machine in xhci.c to
see if a more robust error-handling path can prevent the permanent detachment.
