My problem was that the FreeBSD Doc Project Primer did not mention the FreeBSD extensions when discussing how to set up the SGML_CATALOG_FILES environment variable. Section 3.2.1, examples 3-6 and 3-7 should at least mention that you'll need to add /usr/doc/share/sgml/catalog to SGML_CATALOG_FILES. I agree that these extensions should not be separated from the doc tree - it sounds like a recipe for trouble.
