I read Linux 2.6.26.3 codes(tg3 driver, it's support only BCM5761), but I found many quirk:-(. So I gave up to support this family. According to my BCM5764, BGE_CHIPID(BGE_CHIPID_BCM5764) is 0xf1000000, PCI Vendor ID is 0x14e4 and PCI Device ID is 0x1684.
