It seems like your drive ad2 is bad. Can you please check your drive using S.M.A.R.T. (sysutils/smartmontools) for a fault? Either that, a broken ata controller or an ata driver bug might have caused your panic. You may want to play with DMA timeouts but often it doesn't help. One possible workaround is to disable ATA-DMA completely but it will reduce performance _a lot_.
