Well, all I have is a set of notes scratched on a sheet of paper.  The 
DMA and IRQ of the card are set from within Windows, if I remember 
correctly.  The default is IRQ 11 and DMA 0.

The I/O addresses are set as follows by the jumpers on the upper 
lefthand side of the card:

0x530           0x604           0xE80           0xF40  
X-X O           O X-X           O X-X           X-X O
X-X O           O X-X           X-X O           O X-X
