Problem described below turned out to be caused by pcic0 being in polling
mode. Assigning a management irq to pcic0 resolves this issue.

(Now I just have to figure out why pccardd doesn't detech card
ejections/insertions when a management irq is assigned. Cards do get
attached during bootup.)
