Did you rebuild a kernel with more file resources? utmp is a bad file to lose updates to as some breakins show up there. Look at the LINT file in /usr/src/sys/i386/conf and add the server-tuning stuff like MAX_USERS. I know you may not have many users, but these are multiplied by a constant to scale the file buffers.
