I have used the Fore PCA200E in FreeBSD 3.2 (with a patch), 3.3
(unmodified), 3.4 (unmodified), and 4.0 (unmodified).

I run CLIP and/or SPANS on my ATM network, so my kernel config is:

options         ATM_CORE                #core ATM protocol family
options         ATM_IP                  #IP over ATM support
#options        ATM_SIGPVC              #SIGPVC signalling manager
options         ATM_SPANS               #SPANS signalling manager
options         ATM_UNI                 #UNI signalling manager
#device         hea                     #Efficient ENI-155p ATM PCI
device          hfa                     #FORE PCA-200E ATM PCI

Also, don't forget that you need to put the proper version of the
microcode into /etc.  Here is the version I use:

8 hole> ls -l pca200e.bin 
-rw-r--r--  1 root  wheel  40984 Oct 29  1999 pca200e.bin
9 hole> md5 pca200e.bin
MD5 (pca200e.bin) = 86c3f62566483ba357e1228fbfb3ec62
