Fresh information (well, fresh to me, since everyone seems to have
forgotten it):

Satoshi removed the 40upgrade.tgz package on 11-11-2000.  There
is some indication (after talking with obrien and conducting an
extensive mailing list search) that someone bumped libc
(libc.so.3 --> libc.so.4) in that upgrade package and that is why
it was removed.  

However, there is still no real explanation, and more importantly,
no replacement for the missing package.
