As to why it didn't hear from 207.158.72.11, one would first check ns-a.lerctr.org logs to see if the NOTIFY was sent to all the six slaves. Then check the log for the same time on ns-b.lerctr.org. Note NOTIFY messages are queued for up to 5 minutes.
