Also you made it all to mo_sta_state(NOTEXIST). That smells like a bss_conf update removed the sta for us. I've hit this before. Maybe only with pre-22000 cards? Test case (likely to reproduce): get into RUN take your AP away wait for connection loss to happen that will trigger the RUN -> INIT newstate change. Things will proceed from that. My 8265 is currently buried somewhere but I'll go and give it a try in a few days hopefully.
