On our PPro box, 64Mb RAM & 150Mb swap, we are seeing this message come up frequently. The machine gets totally locked up. We are running 2.2.5 and Apache 1.3.1. It has been stable for several months but the last couple of days there has been a spike in web traffic. Is it a RAM or a swap problem? There is some discussion of this on the mailing list archives, however, no one has posted a workaround yet.
