I recommend to look at RRD and the front end Cricket.

MRTG has a hardtime without modifications to get all the information
from SNMP to tell you for example which exact port you are monitoring
(At least true for the Catalyst 4000, 5000, 6000 series, not true
to the 2900XL series).
