The rules were not loading because hostnames had crept into my pf macros. The hostnames failed to resolve at load time (because DNS was not ready). Once I fixed that problem, the rules loaded at boot time. The error messages provided were from a 6 day old screenshot which I incorrectly assumed was still a problem. It was outdated.
