Nowadays there are PCI Express cards with more than one output, and that's what I did last time (for Monty Widenius). That was an NVidia 6600 card with two outputs. There may be more. Otherwise you will probably have to add normal PCI cards, which are difficult to come by nowadays. You might be better off getting a used top-of-the-range card made before AGP became mainstream. How many? As many as you have cards, slots, monitors and space on your desktop. If the slots are the limitation, you can spread it across multiple machines with x2x (in the Ports Collection). I'm currently running 7 monitors on 4 machines.
