My guess:

Leave ROM off, MEM doesn't matter, I/O is your base address.. and
0x300 is sure to be one of them. You can either

 (a) Try all jumper combinations with kernel set to 0x300
 (b) Try a bunch of kernel settings (boot -c) to see if you
     can find the card.

If you do (a), it looks like it will only take <= 16 attempts.. :-)
