I've had significant recent experience with MIMEdefang and it does a great job, but it's certainly not the only solution in the world. The handling of Perl slaves is pretty well thought out in MIMEdefang, probably its strongest feature. There is the MIME decoding part too, which makes it easy to add your own custom filters to work over complex MIME messages. -- "Where am I, and what am I doing in this handbasket?"
