Using a SATA Promise PDC40718 controller with FreeBSD 8.2-Release and 8.2-Stable causes the following messages to appear:

The relevant disk has been replaced 3 times. Smartmon tools have shown no errors.
The controller is listed as being supported in:

However, there is a PR#116935 indicating that it should not be used.

There is really no way to repeat the problem. It just happens after a period of approximately 2 months without any other indication.
