When I visited the US (OK, it was 19 years ago) it always seemed
to be called the first floor.

I presume you mean in the context of "...knocked on the door and
woke me up". It also has another meaning, although the woman
concerned would be unlikely to describe it that way.
Of course the phrase "Can I bum a fag" *most definitely* has
different meanings here and there ;-)
